Before Your Appointment
Sickness Policy – Elite Health & Beauty recognizes that both massage therapists and clients are vulnerable to infections and therefore ask clients to cancel appointments when they are feeling unwell. If you have any of the following contagious illnesses the massage session will be rescheduled:
Diarrhea, Vomiting, Fever, Chicken Pox, Measles, Mumps, Meningitis, Hepatitis A, Conjunctivitis, Rubella, Head Lice, Impetigo, Influenza, Meningococcal Disease, Polio, Ringworm of the body, feet or scalp, Scabies, Thrush, Whooping Cough, and the Common Cold.
First-time visitors, please arrive 5 to 10 minutes early to your appointment for intake and consultation. Late arrivals may result in loss of session time due to the following appointments. Please consider traffic, locating the location, and parking.
New Client, New Service Intake: If this is your first visit, receiving a new service, or it has been more than 6 months since your last visit, please complete an intake form for the service you are visiting for. Forms are located on the scheduling page.
130 Bates Ave SW: Parking: Rear parking lot of Certi-Pay or street-side parking. Enter through the parking lot or front entrance located on the corner of Bates Avenue and 2nd St., Southwest.
412 Longfellow: Upon arriving if you are more than 15 minutes early the entrance may be locked if in session. The door will be opened 10 minutes before the appointment.
When scheduling an appointment please schedule with the name and contact information of the person receiving services so they may be contacted with appointment information.
If you are required to pay a cancelation, reschedule No-call No-show, or service deposit, please do so before scheduling your appointment. Include in the “notes to the therapist” section that your fee has been paid. If your account is flagged for fees that are not paid before booking an appointment. your appointment will be canceled.
When booking appointments if you are redeeming a voucher or gift certificate/ card, please include the voucher number or code in the “Note to Therapist” section along with any other information you would like the therapist to know. Relaxation, Neck/Shoulder, Lower back, Right arm pain.

Ex: Full body with a focus on low back and left hip/leg (sciatica). Also, have some Neck/shoulder tightness. This will help better prepare the therapist for your appointment.
